Job description
Therapy/Rehabilitation - PT Home Health
Minimum Experience: 1 year; 1st Timers accepted
Patient Population: Adults to Geriatrics
Certifications / Licenses Required: BLS, GA PT License or Compact PT OK
General Skills and Work Setting Requirements: Home Health PT – Geriatrics, Strokes and Orthopedics
Ratio / Census: 7 visits; Full Time Productivity is around 25 units
Unit Systems: Home Care Home Base (HCHB) highly preferred. Transitioning to EPIC around September 2026. If the clinician doesn't have experience with Homecare Homebase (HCHB), they will be required to complete an online HCHB course prior to starting. The course is $250 and must be paid by traveler upfront.
