Key Responsibilities
Evaluate and treat patients with speech, language, cognitive, and swallowing disorders in a skilled nursing facility setting
Develop and implement individualized treatment plans based on patient needs and goals
Monitor and document patient progress, adjusting treatment plans as appropriate
Collaborate with interdisciplinary care teams to ensure comprehensive patient care
Educate patients, families, and caregivers on therapeutic techniques and home programs
Required Qualifications
Active Speech Language Pathologist (SLP) license in the state of New Jersey
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or eligibility required
Experience working in a skilled nursing facility or similar clinical setting
Strong clinical documentation and communication skills
Preferred Experience / Skills
Prior experience in a SNF or long-term care environment
Familiarity with dysphagia evaluation and treatment protocols
Experience with MDS/Medicare documentation requirements
