Job description
This clinician provides telehealth follow-up medication management for older adults, with a focus on geriatric mental health, cognitive concerns, and medication safety. Visits center on depression, anxiety, dementia, mild cognitive impairment, behavioral symptoms of dementia, sleep issues, and medication-related problems. Care is team-based, with nursing support for ongoing monitoring and coordination.
Key Responsibilities
Conduct telehealth follow-up visits for psychiatric medication management in geriatric patients
Monitor treatment response and adjust medications with attention to polypharmacy, fall risk, and medical comorbidities
Perform cognitive screenings and assessments using tools such as MoCA and MMSE
Develop initial care coordination and behavioral support plans
Collaborate with nursing staff, administrative support, and supervising psychiatry leadership
Maintain timely and accurate documentation in the EMR
Support care for patients in assisted living, long-term care, and community settings
Required
Active Pennsylvania license as a Physician Assistant or Psychiatric Mental Health Nurse Practitioner
Licensed and certified to practice in Pennsylvania
Comfort delivering care via telehealth
Preferred
Experience in geriatric psychiatry, long-term care, or dementia care
Experience with cognitive assessment tools and behavioral management strategies
Strong clinical judgment related to medication safety in older adults
Experience working with older adults or in geriatric mental health
Bilingual English/Spanish skills
