Transition Case Manager

Full Time Raleigh, NC, US 1 Attachments

Salary: $40,310.40 Annually

Role Summary

The position works directly with Alumni and CTR II participants to support employment, housing stability, financial self-sufficiency, and successful community transition. The position provides case management and resource coordination, maintains community partnerships, and assists with the management and day-to-day operations of the Forward Together Housing program.

Role Requirements and Preferences

Required:

Minimum of 2 years of sustained recovery and demonstrated knowledge of recovery principles.

Valid driver license and transportation.

Completion of a High School Diploma or equivalent (preferred.)

One (1) year of relevant experience, or any equivalent combination of education and experience that demonstrates the ability to do the tasks required.

Must have the ability to define problems, collect data, establish facts, draw valid conclusions and engage in ethical decision-making.

Core Responsibilities

Coordinate CTR II team meetings and support communication regarding participant needs, progress, and program activities.

Work collaboratively with other departments to address issues and coordinate services related to participants.

Communicate relevant participant needs, barriers, progress, and outcomes with appropriate team members while maintaining confidentiality.

Support program activities and organizational initiatives as assigned.

Review applications for Women’s and Children’s Housing and collaborate with the Child and Family Case Manager to determine eligibility.

Maintain the application pool for interested residents.

Assist CTR II participants in securing employment and housing.

Coordinate and track resident rental payments.

Maintain rent deposits and rent collection for CTR II participants.

Provide case management and continuing support for Job’s Journey tenants.

Participant Support & Case Management

Make appropriate referrals based on the goals and needs of CTR II participants.

Provide ongoing support to all alumni as needed.

Serve as the point person for participants in Retrack.

Work closely with and support other areas regarding issues related to all participants.

Assist participants with obtaining identification, Social Security cards, criminal records, and credit reports.

Provide ongoing support and case management services to promote participant stability and self-sufficiency.

Employment, Education & Self-Sufficiency

Assist CTR II participants in developing employment and housing stability.

Advocate for participants with employers, landlords, and other agencies.

Help participants develop money management skills to support successful self-sufficiency.

Collaborate with the Life Skills Instructor on employment, education, and skills-training opportunities and activities.

Community Partnerships & Advocacy

Establish, foster, and maintain relationships with community partners.

Attend relevant community meetings and trainings.

Advocate for participants with employers, landlords, and other agencies.

Program Coordination & Facilitation

Orient participants to CTR II.

Assist with and facilitate Refocus groups and the CTR II community.

Provide on-call coverage.

Documentation & Outcomes

Document participant and resident progress and outcomes.

Track 12-month alumni outcomes through CRM software or the EHR.

Leadership & Professional Responsibilities

Serve as a role model for participants, residents, volunteers, and Peer Mentors.

Adhere to all policies and procedures of Healing Transitions.

Perform other duties as assigned.

Language Requirements

English is the primary language of software, documents, participants, and staff of HT, though speaking additional languages is a valuable asset. Must have the ability to read, analyze, and interpret common documents; the ability to respond to common inquiries or complaints from participants, or members of the community; and the ability to effectively present information to top management and public groups.

Travel Requirements

This position requires travel in between both facilities and to community engagements and/or meetings.
