Transition Coordinator

Teaching & Learning Tunica County School District

Qualifications: 1. Associate Degree or Higher 2. Such alternatives to the above qualifications as the Board may find appropriate and acceptable.

Supervisor: School Principal/Special Education Teacher

Job Goal

The Transition Coordinator is instrumental in preparing students with disabilities for post-secondary life, including higher education, employment, and independent living.

Duties and Responsibilities

Individualized Transition Planning

Identify transition inventories to be used.

Develop and implement individualized transition plans for students with disabilities, ensuring alignment with their post-secondary goals.

Assist teachers with the development of transition goals and activities for the classroom and the IEP.

Assist teachers with the completion of transition assessments yearly and assist with the completion of transition portfolios for students graduating or exiting school.

Career and Academic Counseling

Provide career guidance and academic counseling to help students understand their strengths, interests, and potential career paths.

Assist students in selecting appropriate courses and extracurricular activities to support their career goals. Identify/locate/secure sites for future job placement/apprenticeships/volunteering/internships

Assist with the completion of transition portfolios for students exiting or graduating from school

Work with High School counselors in providing Transition Services for students with disabilities in General Education.

Work-Based Learning Opportunities

Coordinate work-based learning experiences such as internships, job shadowing, community trips, and vocational training.

Establish partnerships with local businesses, community organizations, and vocational programs.

Identify/locate/secure sites for future job placement/apprenticeships/volunteering/internships.

Oversee and train students placed on job sites/volunteer sites/etc.

Coordinate transportation for students from school to work or volunteer sites and job fairs.

Evaluate students while placed on work sites.

Resource Coordination

Connect students and families with community resources, support services, and post-secondary education options.

Maintain updated information on local, state, and federal resources available for students with disabilities.

Coordinate services with MS Vocational Rehabilitation Services

Coordinate services with local community colleges

Coordinate services with other transition providers and employers

Skill Development

Facilitate workshops and training sessions on life skills, job readiness, self-advocacy, and independent living skills.

Provide ongoing support to help students develop the necessary skills for successful transitions.

Teach transition related skills in small groups (resume writing/interview, etc.).

Assist teachers/parents/students in obtaining: Birth Certificate, Driver's License, Social Security Cards, Tax Preparation Information, Voter Registration Cards

IEP Meetings and Documentation

Participate in Individualized Education Program (IEP) meetings to contribute to transition planning and goal setting.

Ensure accurate documentation of transition services and progress in accordance with legal and school district requirements.

Family and Community Engagement

Engage families in the transition planning process, providing information and support to ensure their involvement.

Foster strong relationships with community agencies and organizations to support students' transition needs.

Training and Professional Development

Stay informed about current best practices and trends in transition services for students with disabilities.

Available to work scheduled hours outside of regular work/school day, if needed.

Participate in ongoing professional development and training opportunities.

Advocacy

Advocate for the needs and rights of students with disabilities within the school system and the broader community.

Promote awareness and understanding of transition-related issues among school staff and community partners.

Maintain open lines of communication with principals, teachers, pertinent staff, and parents

Participates in professional growth activities through an ongoing program of job-related knowledge and skill development.

Data Collection and Reporting

Collect and analyze data related to student outcomes and transition services.

Prepare reports for school administration and stakeholders to demonstrate program effectiveness and identify areas for improvement.

Provide data for Indicator 14.

Maintain compliance with the requirements as outlined by MDVRS and MDE.

Maintain transition checklist.

Performs other related tasks as assigned by the Special Services Director and/or Superintendent.

Terms of Employment: Salary and work year will be established by the Board.

Evaluation: Performance of this job will be evaluated in accordance with provisions of the Board's policy on Evaluation of Professional Personnel.
