Job description
Southwest Network is seeking a Transition Facilitator to coach and support young adults (16.5–22) as they move from youth to adult services. You will guide them in employment, education, housing, finances, and daily living using TIP principles.
The role requires collaboration with multiple partners across health, education, and welfare systems, plus administrative duties. Flexible hours and community-based work are expected to meet the needs of the program participants.
