Job description
Harris Health in Houston is seeking a Clinical Nurse Care Manager I to support the healthcare team in transitional care, acting as a liaison between patients, families, physicians and providers to improve outcomes and satisfaction.
You will manage a patient caseload, coordinate care across the continuum, and contribute to performance improvement while upholding ServiceFIRST standards. A BSN, Texas RN license, and 3 years RN experience are required; CM/QM/Coding experience and BLS are preferred.
