nWe are currently seeking a Transitional Counselor. The Transitional Counselor provides high-quality re-entry services to people returning from NYC jails. This includes assessment, service planning, referrals, case management, group facilitation, and other services as needed. The Transitional Counselor may go into jail facilities or reentry hotels or do other fieldwork. This position maintains progress records and provides written reports and statistics to funding sources and other regulatory agencies. This position will be working hybrid between the office, remotely, and Riker Island

nThis position requires clearance by the New York City Department of Correction. Candidates must be able to obtain and maintain Department of Correction (DOC) clearance to work on Rikers Island and/or other applicable facilities operated by the Department of Correction.

nSalary: $27.80 - $28.84 hourly

nRequirements

nEssential Duties:

nnnOffer outreach to participants prior to and following discharge to engage them in community-based program services

n

nnProvide discharge planning and engagement services inside of NYC jail facilities on Rikers Island or community-based jails

n

nnComplete a full psychosocial needs assessment

n

nnDevelop individual service plans with the participant, identifying specific services and providers for each identified need

n

nnSource referrals and applications to obtain identifying documents, benefits and other entitlements, housing, medical care, behavioral health care, employment, education, family services, and services to address other needs that arrive

n

nnProvide ongoing case management, contacting participants at least once per week in the first 30 days and at least 2 times per month thereafter

n

nnDocument all contacts and services as well as assessment, intake, service planning, and referrals in ECM promptly, within 24 hours of service provision

n

nnEngage in employment retention services as part of wrap-around case management services to employed participants

n

nnMaintain fidelity to evidence-based curricula and cognitive-behavioral interventions

n

nnWrite and submit court letters indicating the progress of participants in these services as needed

n

nnAttend and participate in interdepartmental, subcontractor, and cross-functional meetings to increase awareness of services as needed

n

nnFacilitate group services/activities (i.e., Job Readiness, Financial Literacy, Anger Management, Parenting, Relationships, Substance Abuse Relapse Prevention, Creative Writing, etc.) as needed.

n

nnComplete admin tasks such as maintaining files, making photocopies, edits, and faxing documents as needed

n

nnAttend required training

n

nnIn-person attendance is required to collaborate with co-workers

nnPerform other duties as assigned

n nMinimum Qualifications:

nnnAssociate’s Degree in Criminal Justice, Social Work, Public Administration, or Human Services; In lieu of a degree, high school diploma or equivalent with two additional years of case management and group facilitation experience in the human services field

n

nnTwo years of direct care experience in an individual and group setting

n

nnTwo years of case management and group facilitation experience required

n

nnExperience working in correctional facilities and re-entry settings preferred

n

n nKey Competencies:

nnnAbility to travel locally

n

nnExcellent interpersonal and problem-resolution skills are necessary

n

nnProficiency in Google and Microsoft Suite

n

nnAbility to work effectively under tight deadlines, pay strong attention to detail, and time management skills

n

nnAbility to work in teams and collaborate effectively with people in different functions

n

nnAbility to take the initiative and drive for results

n

nnAbility to communicate with all levels of the organization and build strong relationships

n

nnBilingual Spanish speaking individuals are a plus.
