Job description
UT Southwestern Medical Center in Dallas is seeking a Transitions of Care Clinical Staff Pharmacist to ensure patients are prepared for transitions to different care levels with safe, well-communicated medication regimens.
The pharmacist will review dosing, indications, and discrepancies with the primary team, and provide counseling and education to patients about medications, uses, and potential side effects.
