Transplant Coordinator
nPosition Summary
nAt Stony Brook Medicine, a TH CTS Senior Transplant Specialist /Transplant Coordinator is a valuable member of our team, who provides clinical services to our patient population. Qualified candidates will demonstrate superior patient care and possess outstanding communication skills while adhering to our high standard of excellence.
nDuties may include the following but are not limited to:
nnnWorking with pre-transplant multi-disciplinary team to conduct new patient evaluations and re-evaluations of patients on annual basis.
n
nnCoordinates patient entry and introduction to the transplant program.
n
nnCoordinates pre-transplant education for patients and their caregivers.
n
nnWorks with the outreach coordinator to provide professional education for community nephrologists and dialysis center staff.
n
nnCoordinates medical, surgical, financial, psychosocial, nutrition, and pharmaceutical evaluation and on-going care needs of pre transplant patients.
n
nnParticipates in patient care planning and selection conference.
n
nnLists patients for transplant in a timely manner to facilitate access to donated organs.
n
nnHelps to maintain the UNOS waiting list according to program policies and procedures, regulatory requirements, and national guidelines.
n
nnRemains knowledgeable of all listed patients' status to ensure patient safety and facilitate organ placement in collaboration with transplant physicians.
n
nnWorks with multidisciplinary team on performance improvement projects.
n
nnAssessment and Management of potential living donors.
n
nnCoordinate the pre living donation education for patients.
n
nnCoordinates the medical, psychosocial and financial evaluation of living donors.
n
nnAttending meetings, national, regional, local and internal.
n
n nQualifications
nRequired :
nnnNYS licensed RN.
n
nnKnowledge of Office/EXCEL.
n
nnExcellent communication skills.
n
nnCritical care/ICU experience as an RN
n
n nPreferred :
nnnCCTC (Certified Clinical Transplant Coordinator).
n
nnExperience with ESRD/kidney transplant.
n

nPosting Overview : This position will remain posted until filled or for a maximum of 90 days. An initial review of all applicants will occur two weeks from the posting date. This means that when the Hospital is faced with an institutional emergency, employees in such positions may be required to remain at their work location or to report to work to protect, recover, and continue operations at Stony Brook Medicine, Stony Brook University Hospital and related facilities.

n nPrior to start date, the selected candidate must meet the following requirements:
nnSuccessfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services
n nnnnComplete electronic reference check with a minimum of three (3) professional references.

n nnSuccessfully complete a 4 panel drug screen
n nnnnMeet Regulatory Requirements for pre employment screenings.
n n
n nnProvide a copy of any required New York State license(s)/certificate(s).
n nFailure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.
n*The hiring department will be responsible for any fee incurred for examination .

nAnticipated Pay Range:
nThe salary range (or hiring range) for this position is $98692 - $ / year.
nThe above salary range represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate's validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.
