Job description
JCCA in Pleasantville, NY is seeking a compassionate Case Planner for its Residential Treatment Campus to support youth ages 5–21 and their families. You will work within a multidisciplinary team to develop individualized plans emphasizing safety, well-being, family connections, and permanency.
You’ll coordinate assessments, participate in team meetings, and advocate for timely discharge and community linkages, while maintaining documentation and compliance with regulatory standards.
