United Staffing Solutions is seeking a travel nurse RN Case Management for a travel nursing job in Peoria, Illinois.

Position: Case Management RN - LTAC and inpatient rehab
Schedule: Monday-Friday, 8:00 AM-4:30 PM, 8-hour shifts; no holidays required
Experience: At least 2 years, preferably acute hospital discharge planning and care coordination
Required: BLS (AHA) and an Illinois RN, LSW or LCSW license; negative drug screen for all panels
Preferred: Long-term acute care or rehab background; MCG is used for utilization review but training is provided
EMR: EPIC
Unit: LTAC and inpatient physical rehab; caseload 13-25
Duties: Both utilization review and discharge planning - interdisciplinary rounding, family meetings, insurance authorizations and concurrent reviews, advance directives, DME, home health and outpatient referrals, transportation and post-discharge resources; develop a plan of care for each assigned patient from admission to discharge and manage throughput and length of stay
Note: LTAC patients are complex - vents, airways, IV antibiotics, respiratory and wounds; rehab is mostly neuro, amputation, cardiac, trauma and complex medical/surgical. Scrubs or business casual, no set color
