Job Summary
nThe organization is seeking a Clinical Pharmacist for Ambulatory Care in a primary care setting. This position involves providing pharmaceutical care across two clinics, with a focus on managing medication therapy and patient-centered services during weekday daytime hours.
nJob Details
nnPosition Description: Ambulatory Care clinical pharmacist responsible for seeing approximately 16 patients daily as a mid-level provider, with responsibilities split between two primary care clinics.
nCertifications and Education Requirements: BCACP board certification and PGY2 Ambulatory Care residency required.
nSchedule: Dayshift, weekdays only, shift duration of 12 hours, minimum of 36 hours per week.
nStart Date: 10/12/2026
nEnd Date: 12/26/2026
nRequired Qualifications: Currently licensed pharmacist with BCACP certification, PGY2 Ambulatory Care residency completion, and at least 1 year of experience in outpatient pharmacy settings.
nAdditional Information: The role involves onsite patient consultations and pharmacy services within primary care clinics, contributing to medication management and patient outcomes.
