Supplemental Health Care is seeking a travel Home Health Physical Therapist to provide patient-centered care in Granite City, Illinois and surrounding areas. The role involves 40 hours per week, 8-hour shifts, and a 26-week travel contract.
The ideal candidate will hold an active PT license in the state, be BLS certified, and have 1+ year of home health experience, with proficiency in MS-DS like OASIS-E, and EMR systems such as Homecare Homebase, NetSmart, or Epic.
