Job description
Supplemental Health Care is seeking a Home Health Physical Therapist for a travel assignment in Bardstown, Kentucky. You will work 40 hours per week for 13 weeks, performing PT evaluations, planning care, and delivering skilled interventions in patients' homes.
The role requires an active PT license in the assignment state, current BLS, and travel between patient homes. EMR experience and knowledge of OASIS-E are preferred. Generous benefits and travel support are included.
