Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Chapmanville, West Virginia. You will provide compassionate, patient-centered PT services in patients’ homes, performing initial and ongoing assessments and developing individualized plans of care.
Expect 40 hours per week with 8-hour days, travel between patient homes, and collaboration with a multidisciplinary team.
