Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Houghton, Michigan. The role requires 40 hours per week, with an 8-hour shift and travel between patient homes.
You will perform PT evaluations, develop plans of care, and collaborate with a multidisciplinary team to deliver patient-centered home health care. Qualified candidates have an active PT license, BLS, and 1+ year of home health experience, plus EMR experience.
