Travel Home Health Physical Therapist job in Fort Morgan, Colorado: Start your assignment in Fort Morgan, CO and help patients regain mobility and independence in their homes. You will review medical histories, assess movement, develop individualized care plans, and use exercises and hands-on therapy to ease pain and improve function. Responsibilities include documenting progress, educating patients and families, and collaborating with healthcare professionals. Required qualifications include a current Colorado Physical Therapy license and a Doctor of Physical Therapy degree. Recommended skills are strong clinical knowledge, adaptability, and excellent communication. Experience in home health is preferred.
