Minimum Requirements
Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
Minimum 1 year of experience required
Home health experience required
Charting experience with Homecare Homebase is required
OASIS-E experience required
Driver's License and proof of insurance required
Experience with adult and geriatric patients required
Experience with chronic disease management required
Experience with wound care, IV, pleurx, and wound vac required
Assignment Details
Shift Hours: 0800 - 1700
Standard Hours Per Week: 40
Approximately 6 total points per day
Adults only patient population
May be asked to float within the service area based on patient needs
