Travel Home Health Physical Therapist – Washington, D.C.
Travel assignment in Washington, DC. Start your assignment in Washington, DC and help patients regain mobility and independence in their homes. You will assess patient abilities, develop individualized treatment plans, implement therapeutic interventions, and educate patients and families on safety and home exercise programs. Responsibilities include evaluating the home environment, documenting progress, and collaborating with a multidisciplinary team to ensure optimal outcomes.

Travel Home Health Physical Therapist job for 13 weeks, 40 hours per week, 8‑hour shifts. Employment Type: Travel.
Responsibilities
Assess patient abilities, develop individualized treatment plans, implement therapeutic interventions, and educate patients and families on safety and home exercise programs.
Evaluate the home environment, document progress, and collaborate with a multidisciplinary team to ensure optimal outcomes.
Required Qualifications
Graduation from an accredited physical therapy program.
Current Washington, D.C. PT license.
Minimum of 1 year home care experience.
Proficiency in OASIS E documentation.
Experience with HCHB/POINTCARE.
Preferred Qualifications
