Travel Physical Therapist (PT) – Home Health
Largo, Florida
We are seeking a Travel Physical Therapist for a Home Health contract in Largo, Florida . This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to adult and geriatric patients in their homes.
This is a great fit for a PT who enjoys autonomy, patient-centered care, and helping individuals improve mobility, strength, safety, and overall independence in their home environment.

Responsibilities
Perform comprehensive physical therapy evaluations and reassessments.
Develop individualized treatment plans based on physician orders and patient goals.
Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
Provide gait training, balance training, transfer training, therapeutic exercise, fall prevention, and functional mobility interventions.
Educate patients and caregivers on home exercise programs, safety strategies, and proper equipment use.
Complete timely documentation and maintain home health compliance standards.
Manage a daily home health caseload, with skilled visits expected in the field.
Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
