Home Health Physical Therapist jobs in Rapid City, SD let you provide care to medically compromised patients with diagnoses like CHF, COPD, DM, cardiac, ortho, neuro, dementia, and general weakness. You will evaluate, plan, and deliver therapy to adults and geriatrics in their homes, using EPIC for charting. You must have at least 1 year of PT experience, SD licensure, and BLS certification.
