We are seeking a Travel Physical Therapist for a Home Health contract in Medford, New York . This opportunity offers the independence and flexibility of home health while providing meaningful one-on-one care to adult and geriatric patients in their homes.

Responsibilities
Perform comprehensive physical therapy evaluations and reassessments.
Develop individualized treatment plans based on physician orders and patient goals.
Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
Provide gait training, balance training, transfer training, therapeutic exercise, fall prevention, and functional mobility interventions.
Educate patients and caregivers on home exercise programs, safety strategies, and proper equipment use.
Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
Complete timely documentation and maintain home health compliance standards.
