Job Title: CWHH-Physical Therapist Location: Pollocksville, NC Setting: Home Health Pay Range: - Schedule: Discuss with recruiter. Contract Length: 13 weeks Candidate Type: Local and traveler friendly. Requirements: Current State License as a Physical Therapist - Required; Proof of current CPR certification - Required; Current driver's license - Required; Must have reliable transportation and insurance - Required; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required; Previous home care experience - Preferred
Responsibilities
Perform clinical assessments to identify patient needs and appropriateness of care.
Document, review, and complete a Plan of Care (POC), progress notes, and other clinical records; revise as needed based on ongoing assessment data.
Plan and deliver treatment for neuromuscular, neurological, and orthopedic disorders based on referrals and assessments.
Instruct patients and families in transfer techniques and equipment use, ensuring safety and proper technique.
Educate patients and families about therapeutic procedures tailored to their age, condition, and learning readiness.
Supervise and coordinate patient care activities with interdisciplinary team members.
Apply infection control protocols to prevent contamination and disease transmission.
Accept clinical assignments aligned with education and competencies.
