Responsibilities
Perform comprehensive physical therapy evaluations and reassessments in patients' homes.
Develop individualized treatment plans focused on improving mobility, strength, balance, and independence.
Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
Complete timely documentation while maintaining quality standards.
