Description
American Traveler is seeking a Home Health Physical Therapist with 2+ years of acute hospital and home care experience and certified home health/OASIS proficiency for a 13-week assignment in Pittsfield, MA.
Details
Home Health setting providing direct patient care in clients' residences
Day shift, 8-hour shifts from 8:00 AM to 4:30 PM
Rotating weekends and holiday coverage required if needed
No on-call requirements
Uses rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL equipment
13-week assignment length
Requirements
MA Physical Therapist license required if applicable
Active BLS certification (AHA) required
2+ years of acute hospital-based nursing care and home care experience required
Certified home health experience with proficiency in OASIS required
Valid driver's license required
Must have own vehicle for home visits
Must be able to work independently with minimal supervision
Acute care experience preferred
Must be able to pass a fit test using an N95 mask (PAPRs not available for home health use)
One supervisory signed reference from within the past two years required
Skills checklist for Physical Therapist current within 1 year required
Additional Information
Responsible for providing physical therapy services and direct patient care to home health clients
Collaborates with interdisciplinary team members in developing and executing the patient's comprehensive plan of care
8 hours of non-billable orientation provided
Candidates residing within 50 miles of the facility will not be considered
Mileage reimbursement provided only to/from the TOR office and/or drop/satellite office, not from traveler's home
No more than 5 days of requested time off will be accepted, and all RTO must be confirmed prior to submission
