Candidate Type: Traveler
Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting.
Responsibilities:
Perform clinical assessments to identify patient needs and appropriateness of patient care.
Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation and ongoing assessment data.
Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments.
Assist/instruct patients and families in transfer techniques appropriate to patient abilities and instruct on the use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances in a safe manner.
Utilize appropriate therapy equipment specific to patient diagnosis and needs, order rehabilitative equipment as needed, and adapt equipment to meet home limitations.
Provide patient and family education regarding therapeutic procedures based on the patient's age, condition, culture, and learning readiness.
Supervise and oversee team members performing patient care activities, ensuring they are consistent with patient needs and desires.
Apply infection control concepts and universal precautions during patient care activities to prevent contamination and disease transmission.
Accept clinical assignments that are consistent with education and competence to care for patients.
Perform other related duties as assigned or requested.
