We are seeking a Travel Physical Therapist for a 13-week Home Health contract in Roseburg, Oregon . This opportunity offers the flexibility and independence of working in the home health setting while providing meaningful one-on-one patient care to adult and geriatric patients in their homes.
This is a great fit for a PT who enjoys autonomy, patient-centered care, and helping individuals improve mobility, safety, and independence in their home environment.

Responsibilities
Perform comprehensive physical therapy evaluations and reassessments.
Develop individualized treatment plans based on patient needs and physician orders.
Treat patients with orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex conditions.
Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
Complete timely documentation and maintain compliance with home health standards.
