Physical Therapist – Home Health
Job Description
We are seeking a dedicated Physical Therapist for our Home Health department. This role is focused on providing quality care and support to patients in their homes. As a Physical Therapist, you will be responsible for conducting assessments and providing therapeutic interventions while ensuring adherence to best practices and compliance with regulations.
Key Responsibilities
Conduct initial evaluations and assessments of patients.
Develop and implement individualized treatment plans based on patient needs.
Perform start of care visits, recertifications, and resumption of care as necessary.
Manage a typical workload of 5-6 patients per day, adjusting for admissions.
Collaborate with other healthcare professionals to enhance patient care and outcomes.
Provide education and support to patients and their families regarding physical therapy goals and home exercises.
Qualifications
Current BLS certification.
Licensed to practice Physical Therapy in North Dakota.
Experience in Home Health preferred, with OASIS training desirable.
Strong communication and organizational skills.
Ability to work independently and manage time effectively.
Work Schedule
This position typically involves a Monday to Friday schedule from 8:00 AM to 5:00 PM, with weekends and holidays off. There are no on-call responsibilities associated with this role.
Orientation and Support
New hires will undergo a brief orientation period of 1-2 weeks, working alongside experienced team members. Ongoing support will be available through clinical coordinators and peer physical therapists within sister agencies.
Additional Information
Black, navy blue, or gray scrubs are required for this position.
Must possess a valid driver’s license and auto insurance.
Mileage will not be reimbursed, as this is an all-inclusive rate position.
