Job description
Role: Travel Home Health Physical Therapist, responsible for helping patients regain mobility and independence in their homes.
Key Duties: Assess medical histories, evaluate movement, develop individualized care plans, administer exercises and hands‑on therapy, document progress, educate patients and families, and collaborate with healthcare professionals.
Qualifications: Current Colorado Physical Therapy license, Doctor of Physical Therapy degree, at least 1 year of experience, PT (Compact accepted), and proficiency with Oasis and Homecare Homebase documentation. SSN and DOB required, along with one reference.
Skills & Attributes: Strong clinical knowledge, adaptability, excellent communication skills, and experience in home health preferred.
