Job description
Role Overview:
Seeking a travel Home Health Physical Therapist to provide patient care in a home setting. The position involves performing clinical assessments, developing and updating Plans of Care, and delivering therapy for neuromuscular, neurological, and orthopedic conditions. The therapist will instruct patients and families on transfer techniques, equipment use, and therapeutic procedures, ensuring safety and education tailored to individual needs. Supervision of team members, adherence to infection control practices, and performing related duties are essential.
Qualifications & Skills
Current State Physical Therapist license
Proof of CPR certification
Valid driver’s license with reliable transportation and insurance
1-2 years experience in acute care or rehabilitation settings
Responsibilities Include
Conducting clinical assessments and documenting patient progress
Planning and providing appropriate therapy treatments
Instructing and educating patients and families
Supervising team members and ensuring quality care
Applying infection control and safety protocols
Performing additional duties as assigned
