Job description
Seeking a travel Physical Therapist to provide home health care services in Wallingford, CT.
Responsibilities include evaluating patients, managing caseloads independently, conducting home safety assessments, and coordinating discharge planning with interdisciplinary teams.
Qualifications include a Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program, and an active Connecticut Physical Therapist license. BLS certification may be required. Prior experience in home health and documentation skills are preferred.
