Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Lenoir, North Carolina. You will provide patient-centered PT services across multiple home visits, typically 40 hours per week with 8-hour shifts.
Applicants must hold an active PT license in the state, current BLS, and experience with home health settings and OASIS-E documentation. Travel between patients is required and EMR systems such as Homecare Homebase, NetSmart or Epic are used.
