Job description
Role: Travel Home Health Physical Therapist
Provide home health services, including patient evaluations, developing treatment plans, and documenting care in compliance with regulations. Work independently within community and home environments to assist patients in recovering from injuries and managing chronic conditions.
Qualifications & Skills:
Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program. Active Texas Physical Therapist license. BLS certification may be required. Experience in home health is preferred. Must have reliable transportation and the ability to travel within the service area.
Key Duties:
Deliver patient-centered care in a home health setting, perform assessments, create and implement treatment plans, and document progress accurately. Work flexible hours in a community-based environment.
