Job description
Role & Responsibilities:
The Physical Therapist is responsible for assessing referred patients requiring rehabilitation services, developing and implementing individualized treatment plans, and providing skilled interventions based on physician orders. They evaluate patient needs, set realistic goals, document progress, and educate patients and families on therapy methods. Collaboration with interdisciplinary teams to plan and assess patient care is essential.
Qualifications & Skills:
Candidates must hold a Doctorate or Bachelor's degree in Physical Therapy, pass the NPTE, and have graduated from an accredited institution. A minimum of one year's experience, current state licensure or active application, and CPR certification are required. Additional client-specific requirements may apply.
Key Duties
Develop and execute care plans for patients
Provide direct physical therapy services in accordance with physician orders
Assess, modify, and evaluate patient progress towards treatment goals
Collaborate with healthcare team members to optimize patient outcomes
Document patient conditions and educate patients and families
