Job description
We are seeking a travel Home Health Physical Therapist to provide therapy services in a home health setting.
Role: Provide patient-centered physical therapy in a home environment.
Responsibilities: Assess patient conditions, develop and implement treatment plans, monitor progress, and document outcomes.
Qualifications & Skills: Valid license as a Physical Therapist, strong clinical skills, ability to work independently, and excellent communication skills.
Key Duties: Deliver therapy services, collaborate with healthcare teams, and ensure quality patient care.
Work Details: 40 hours per week, 8-hour shifts, travel-based employment.
