Job description
Role: Travel Home Health Physical Therapist
Responsibilities: Provide in-home therapeutic services to help patients recover from injuries and manage chronic conditions, collaborate with interdisciplinary care teams to develop and implement patient-centered care plans, maintain accurate documentation to ensure regulatory compliance, and support patient independence through targeted interventions.
Qualifications & Skills: Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA; active California Physical Therapy license; BLS certification from AHA or ARC (may be required); preferred experience in home health therapy and familiarity with documentation systems. Ability to achieve productivity of 30 visits per week.
Key Duties: Deliver skilled therapeutic services in patients' homes, develop personalized care plans, document care activities, and work within interdisciplinary teams to optimize patient outcomes.
Additional Information: The role involves working five days a week, with a focus on patient-centered care in a scenic location with access to coastal and cultural attractions.
