Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Thomasville, GA. You will perform initial and ongoing PT assessments, develop plans of care, and collaborate with RNs, OTs, SLPs, and physicians to deliver patient-centered home health care.
The role requires an active PT license in the state, current BLS, 1+ year home health experience, and proficiency with EMRs such as Homecare Homebase, NetSmart, or Epic.
