Job description
Supplemental Health Care is hiring a Travel Home Health Physical Therapist to provide patient-centered care in Winston-Salem, North Carolina and surrounding areas. The contract spans 13 weeks, with 40 hours per week and 8-hour days, traveling between patient homes as needed.
The role requires an active PT license, BLS, and at least 1 year of home health experience, with familiarity of OASIS-E and Medicare guidelines; EMR experience with Homecare Homebase, NetSmart, or Epic is preferred.
