Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel job in Antigo, Wisconsin. The role involves performing PT assessments, developing plans of care, and delivering patient-centered home health services across Antigo and surrounding areas.
Must have an active PT license, current BLS, and at least 1 year of home health experience. EMR familiarity with Homecare Homebase/NetSmart/Epic is preferred. Travel between patient homes is required.
