Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel assignment in Lynn, Massachusetts. The 13-week contract offers 40 hours per week, with 8-hour day shifts and travel between patient homes.
You will perform PT evaluations, develop plans of care, and provide interventions for strengthening, gait, balance, and safety. Collaboration with RNs, OTs, SLPs, and physicians is required, and documentation must follow Medicare/OASIS-E guidelines.
