Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel assignment in Hardinsburg, Kentucky. You will provide patient-centered home health PT services on a 13-week contract, typically working 40 hours per week with 8-hour shifts, and travel between patient homes.
The role requires an active PT license in the assignment state, current BLS, 1+ year of home health experience, and proficiency with Medicare OASIS-E guidelines.
