Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Grants Pass, Oregon. The role involves performing PT evaluations, developing care plans, and delivering skilled interventions in patients' homes across the Grants Pass area.
Requirements include an active PT license in the state, current BLS, and at least 1 year of home health experience. EMR experience with Homecare Homebase, NetSmart, or Epic is preferred, with travel between patient homes
