Job description
Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Fort Smith, Arkansas, offering 40 hours per week and 8-hour shifts. The PT will travel between patient homes, perform PT evaluations, and develop individualized care plans in home health settings.
Qualifications include an active PT license, current BLS, 1+ year home health experience, and proficiency with EMR systems such as Homecare Homebase, NetSmart, or Epic.
