Job description
Preferred Home Health Care & Nursing Services is seeking a travel Home Health Physical Therapist to provide patient-centered therapy in a home health setting. Responsibilities include assessing patient conditions, developing and implementing treatment plans, monitoring progress, and documenting outcomes.
You will deliver therapy services, collaborate with healthcare teams, ensure quality patient care, and work 40 hours per week across 8-hour shifts with travel.
