Job description
Generations Hospice, affiliated with BrightSpring Health Services, is seeking a Nurse Practitioner to provide patient-centered hospice care across Fond du Lac, Beaver Dam, Oshkosh and surrounding areas. You will assess patients, manage symptoms, plan care, and coordinate with the IDT to ensure goals align with patient wishes.
The role involves travel within the territory, performing visits, and ensuring high-quality care in a compassionate, interdisciplinary setting.
