Job description
Your Health Organization in the Greenville area is seeking a Registered Nurse to join the patient care team as a Nurse Case Manager. The nurse will visit patients at home, coordinate with providers, and initiate telemedicine visits to support care plans.
Travel is required daily to homes and facilities; a reliable vehicle is needed. This is a full-time, salary-based role with 12-hour shifts and opportunities to contribute to high-quality, compassionate care.
