Job description
Advantmed is seeking a Nurse Practitioner for in-home wellness visits across Southern Michigan. You will perform comprehensive health assessments, annual wellness visits, and risk adjustment documentation to support value-based care.
Requires 3+ years in direct patient care, active Illinois NP licensure, and willingness to travel. Travel ~75% with hotel and mileage support; EMR proficiency and strong patient education skills are preferred.
