Job description
Optum Home & Community Care, part of the Optum family, is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to conduct in-home Medicare assessments across Oregon. This field-based role emphasizes education, gap closure, and collaboration with PCPs.
Position offers flexible scheduling (8am-8pm across 7 days), no on-call, no weekends/holidays; travel across the state with expenses covered; base pay plus incentives.
