Job description
SNF Healthcare is seeking a licensed Nurse Practitioner (NP) or Physician Assistant (PA) to travel to skilled nursing facilities across New York for monthly patient visits. This role offers flexibility and the opportunity to make a direct impact on patient care while working independently.
The ideal candidate will have an active New York NP or PA license, experience in geriatrics or long-term care, and strong documentation and communication skills.
